Medication Shortages: Impact on Medication Errors and Case Reviews of Criminal Negligence

Upon completion of this activity, pharmacists will be able to: Identify root causes of current medication shortages and why the current situation is being recognized as the most severe in history; Review most recent near miss and medication errors reported which are related to/associated with medication shortages; Discuss the RaDonda Vaught vecuronium administration error in place of midazolam resulting in patient death, unintentional error, which resulted in criminal negligence in Tennessee and turned the medical world upside-down.
